Secondary Transition

Secondary Transition section is required to be completed for a student who will be 14 years or older through the data date rage of the current IEP.
1. Select Secondary Transition from the IEP Table of Contents or from the navigation drop-down list in the section title located on each of the IEP page. Complete the Secondary Transition screen with appropriate information. Select the plan in which the student is participating (4 year, 5 year, or 6 year plan)
2. Document student's postsecondary goals. One goal must be indicated for employment and one goal must be indicated for education and/or training. For each Postsecondary Goal a related IEP Goal must be identified. Click the Add/Edit Related Goals button to select the related IEP goal.
4. Select the student's course of study, List of courses the student is taking that align with their Postsecondary Goals.
5. Complete the Projected Date of Exit field and indicate if a copy of both the Maryland Summary of Performance (MSOP) and the A Family Guide to Secondary Transition Planning in Maryland were provided.
Complete the form and click the Audit for Completion button.
